Advanced Caregiver Certification Application

Description

Requirements:

1. Successful completion of the Core Caregiver Training Course
2. Successful completion of at least three Specialization Courses
3. A minimum of 20 hours of general caregiving ministry
4. A minimum of 30 hours of additional caregiving ministry with at least 10 hours in each of the chosen Specialization demographics
5. 20 hours of mentoring with a seasoned ministry leader of which at least 5 of those hours must be related to each of the three chosen Specialization demographics
6. Completed Application with Attestation
7. Submit three references (Personal, Ministry, Ministry)
